What is PolyMyalgia?
Polymyalgia rheumatica (PMR) is an arthritis-related disease characterized by moderate to severe muscle pain and stiffness in the neck, shoulder and hip area. PMR occurs in people older than 50 years, and women are affected twice as often as men.
The pain and stiffness of PMR are caused by inflammation of the joints and surrounding tissues. While the shoulders and hips are most often affected, inflammation may occur in other parts of the body. |

If left untreated, PMR generally subsides or goes away on its own after one to several years. With treatment, the symptoms of PMR are quickly and well controlled, but relapse may occur after treatment is stopped. While the disease does go into remission, it can return.
PMR is related to another inflammatory disease called giant cell arteritis (GCA). Ten to 30 percent of people with PMR will develop GCA as well; so, people with PMR must be familiar with and be on the lookout for symptoms of GCA. Quick diagnosis and proper treatment are necessary to prevent serious complications of GCA, such as blindness.
What causes it?
The cause of polymyalgia rheumatica (PMR) is unknown. It is suspected that both genetic and environmental factors play roles in the development and severity of PMR. The fact that PMR is occurs in people older than 50 years of age suggests it may be linked to the aging process. The muscle pain of PMR is related to the joint pain and the general inflammatory process.
What are the main effects?
The main symptoms of polymyalgia rheumatica (PMR) are muscle pain and stiffness in the neck, shoulders and hip area. These symptoms may also be felt in the low back, thighs and trunk. Stiffness is usually worst in the morning or after a long period of inactivity, but you will loosen up as you start moving around. Pain may wake you at night, and turning over in bed may be difficult.
Typically, disease onset is abrupt. You may go to bed feeling fine and be too stiff and sore to get out of bed the next morning. The muscle aches involve both sides of the body, and the shoulders are usually the first areas affected. Some people’s symptoms will progress more slowly over time. Along with pain and stiffness, the following symptoms may occur:
- Fatigue
- Weakness
- Fever
- Loss of appetite
- Weight loss
- Tenderness over the shoulders, hips, low back and buttocks
- Swelling of the joints in hands, wrists and knees.
- Numbness and tingling in the fingers (carpal tunnel syndrome)
Treatment options?
The symptoms of polymyalgia rheumatica (PMR) can be alleviated with the use of low doses (10 to 15 milligrams per day) of corticosteroids (such as prednisone). Symptoms will usually disappear in 24 to 48 hours. The rapid improvement with steroids is often used by doctors as a confirmation of the diagnosis of PMR. If there is no improvement, your doctor will consider a different diagnosis.
Most people will need to continue taking steroids for one to three years, but some will need to continue taking them for as long as nine or more years. If symptoms recur after discontinuing the drug, steroid treatment will be required again.
Nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, are not enough to treat PMR.
Exercise and rest play important roles in your treatment and recovery. Exercise helps you maintain or regain your energy and muscle strength. Exercise also helps you fight the weight gain and osteoporosis that may result from taking corticosteroids. Good forms of exercise include walking, riding a stationary bike and exercising in a pool. You also need enough rest to give your body time to recover from exercise and other activities. Treatments like massage, acupuncture, and physiotherapy can all soothe pain and improve morale. Note that low-level repetitive circulatory massage can be more effective than other forms of massage to relieve pain on an on-going basis.
